All patients should read and sign the Patient Financial Policy and Form. New patients will be asked to fill out New Patient Registration, HIPAA Release, Acknowledgement Of Privacy Practices, and Minor Consent (if applicable). You may print these forms and complete them prior to your appointment. We look forward to seeing you!
Please understand that payment for services is a part of our relationship. We participate with most major insurance plans. It is your responsibility to know if your plan has limits on the providers you see or services you receive. Depending on your coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. It is your responsibility to notify our office of any patient information changes (i.e. address, name, insurance information, etc.). You are expected to bring your current ID and insurance cards to all appointments.
